Please complete this form for each quilt you are mailing. If you are coming into the shop, you do not need to complete this form online. You may stop in during shop hours or book time in person by calling the shop at 828-595-2244 or texting Rebecca at 215-740-3755.  Please note: Quilts enter my queue in the order that I receive them. If you have a deadline, please check with me on timeframe and note that there will be a $100 charge for rush requests.
